We are looking to add a dedicated and compassionate Care Coordinator to our team. The successful candidate will be responsible for consulting with patients to determine their needs, developing treatment plans and coordinating care services, and working with the care team in order to evaluate the success or failure of the treatment plan in place.

  • BA/BS in social work or a related field.
  • Must be licensed.
  • Minimum of 2 years’ experience in clinical practices or care coordination role.
