- Overseeing care plan and ensuring patient is being administered treatment as needed.
- Follow-up visits, evaluations and general case management for all patients on roster.
- Bachelor’s degree in social sciences, social work, nursing or equivalent.
- Licensed RN (Registered Nurse).
- Recent work experience in case management.
- Licensed CCM (Certified Case Manager) preferred.
- Experience using Healthcare Management software.
- Advanced communication skill.
- Local travel may be required.
