Duties & Responsibilities

  • Responsible for ensuring that all parties – including healthcare provider, family and patient – are kept up to date on information pertaining to case.
  • Maintaining a network of community providers, social support and other resources for regular case outreach.
  • Overseeing care plan and ensuring patient is being administered treatment as needed.
  • Follow-up visits, evaluations and general case management for all patients on roster.


  • Bachelor’s degree in social sciences, social work, nursing or equivalent.
  • Licensed RN (Registered Nurse).
  • Recent work experience in case management.
  • Licensed CCM (Certified Case Manager) preferred.
  • Experience using Healthcare Management software.
  • Advanced communication skill.
  • Local travel may be required.
